Forms not opening sometimes after login

Forms not opening sometimes after login
Sometimes when i click menu links (from self service), the forms does not open. How to check the issue? Which logs etc.?
Application version 11.5.10.2, db 10.2.0.4. All the links doesn't work sometimes like

System Administrator > Concurrent >Request.

Most of the time I have to restart the apps and db and then it starts working. And again users report the same issue.

Dear you have to check the $LOG_HOME and check the apache and concurrent services logs. Is this issue comes on all systems?

2ndly do not restart the db tier just check that if this issue comes then bounce the apache service. after that retest the issue. plz update your system response here.
